All Breed Heart Screening Clinic
Conducted by board certified cardiologist Dr. Nancy Morris and Sue Krom, Director of Cardiology Services, Mass Vet Cardiology Services.Registration
All appointments are on a first-registered and paid basis. Participants MUST pre-register at www.Heartlightdobermans.com. Sue Krom will schedule appointments. Please list under comments on the registration whether you have a conflict with showing or prefer a certain day or time, and Sue will accomadate as much as possible.

A MasterCard/Visa application also available online at www.Heartlightdobermans.com. Complete the application and print and fax to 781-826-7608 if you prefer to pay by MasterCard or VISA.
You will not be considered completely registered until Sue has received payment. Late entries and payments MAY be taken provided that the number limits for the clinic have not been reached.

Sue Krom will contact you via email or telephone to confirm the time of your appointment. Please make sure to provide COMPLETE registration information on Dr. Morris' website.
Cost
You may purchase any single test or all tests. Information collected can be used for the 2008 UDC Breed Survey Test.

Cardio Care Blood Test: $55.00
OFA Heart Registry: Participants must download OFA forms from the OFA website and complete them in advance in order to receive the half price OFA sticker. Please bring the form with you to the clinic. You will send this document directly to OFA along with $7.50.
